For many, ketogenic eating is a term from the world of weight loss. In medicine, however, it has been known for far longer, especially from the treatment of epilepsy. In recent years the focus has broadened: ketosis is increasingly discussed as a metabolic state that may not only influence weight, but possibly also processes connected with ageing.
At the same time, a cool head is helpful here. Animal studies suggest that ketogenic diets can influence lifespan and healthspan. In humans the evidence is much less clear, because there are hardly any long-term studies that genuinely measure lifespan. What can be assessed more reliably are markers such as blood glucose, insulin, inflammation, blood pressure, blood lipids, body composition and certain functional values. That is also where the realistic strength of the approach lies: ketosis can be a tool for some people to improve metabolic health. Whether that automatically means “living longer” is a different question.
What is ketosis, really?
Ketosis is a normal metabolic state. When carbohydrates are scarce, insulin secretion drops and the body increasingly uses fatty acids as an energy source. The liver then produces ketone bodies, mainly beta-hydroxybutyrate, acetoacetate and acetone. Ketone bodies are an alternative fuel that many organs can use, including the brain.

Can keto help with weight loss?
Yes, for many. Reasons include stronger satiety, fewer cravings and clear rules. The early effect on the scales can also be amplified by water loss through glycogen breakdown. That is not false weight loss, but it is worth knowing that the first kilos are not only fat.
Does keto improve blood glucose values?
In people with prediabetes or type 2 diabetes, fasting glucose, HbA1c and insulin resistance often improve, especially when weight drops and overall food quality improves. Medication is important here: anyone taking glucose-lowering drugs often needs adjustments to avoid hypoglycaemia.
What about fatty liver?
Non-alcoholic fatty liver disease is closely linked to insulin resistance and visceral fat. Significant weight reduction, regardless of dietary style, can improve fatty liver. Keto can be one route to that goal, but it is not the only one.
How does keto affect hunger and eating behaviour?
Many report that constant snacking stops and hunger feels clearer. That can bring relief. For some, however, it tips into a very rigid relationship with food. This is an important distinction: keto can help create structure, but it should not lead to compulsion or social isolation.
Which risks and disadvantages should be taken seriously?
A ketogenic diet is not automatically healthy. It can be, when it is well constructed. It can also create problems if it becomes one-sided or permanently extreme.
What happens to blood lipids?
A central point is LDL cholesterol. On keto, triglycerides often fall, HDL frequently rises, which sounds good. At the same time, LDL can rise sharply in some people. This is not the case for everyone, but it is clinically relevant. Anyone planning to do keto longer should therefore have their blood values checked and not look only at weight loss.
Can fibre intake fall short?
When keto is lived as “meat, cheese, butter”, fibre is missing. This can promote digestive problems and is unfavourable for the gut microbiome. Especially in a longevity context, this is paradoxical, because a diverse microbiome and a fibre-rich diet are often considered health-promoting. A well-planned ketogenic diet therefore needs vegetables, nuts, seeds and suitable fibre-rich options within the ketogenic framework.
What about micronutrients?
When entire food groups are heavily restricted, the risk of nutrient gaps rises, depending on food choices. Typical issues are magnesium, potassium, folate and certain trace elements. In addition, water and electrolyte balance change during the transition phase. The famous “keto flu” is often less a mystical detox than an electrolyte and fluid issue.
Is keto sensible long term?
There is no single answer here. For some, keto is a time-limited intervention that later transitions into a moderately carbohydrate-reduced, Mediterranean diet. For others, keto fits long term. What matters is that blood values, well-being, muscle mass, training performance, digestion and everyday practicality remain in good shape. If they do not, it is sensible to adapt the approach rather than force it.
Who is more likely to benefit from keto?

Who is keto less suitable for?

In diabetes treated with insulin or with medications that can cause hypoglycaemia, keto should only be started with medical supervision.

What does a high-quality ketogenic diet look like?
In daily life, a solid version usually looks like this:

- if needed, omega-3 sources, depending on the dietary pattern

Longevity is not only biochemistry but function. Muscle strength, balance and physical capacity are strongly linked with health in older age. Any dietary pattern that promotes muscle loss is problematic in the long run. Protein intake and resistance training are therefore not a side issue but a core point.
Does keto have to be permanent?
